The Healthy by Choice (HBC) program is a voluntary health management program open to eligible non-union employees and their covered spouse/domestic partner, and employees covered by the collective bargaining agreements (CBA) between the City and the USWA, FOP Lodge #9, and the FOP/OLC and their covered spouse.

Healthy By Choice Participation Activities

IMPORTANT: All requirements need completed by September 30, 2016.

  1. Take an online Health Risk Assessment – myuhc.com
  2. Participate in Health Screenings during Open Enrollment
  3. Participate in two (2) wellness educational classes. See schedule here.
  4. Complete a Preventative Care Check-Up and submit the Preventive Care Form signed by your primary care physician.
